School Health Forms
All students must be up-to-date with Medical and Immunization requirements and all completed forms must be turned into your child's school health office by October 15. We recognize that specific medical conditions may affect the scheduling of appointments and recommend that families work with their primary care providers as social distancing practices evolve. If you cannot get an appointment before August 15, you must submit proof of the upcoming appointment.
- Medical and Emergency Information Form - Required for all students annually.
- Medication Authorization Form - Only if needed.
- Illinois State Health Form - Required for all new students, and students entering Kindergarten or 6th grade.
- Illinois State Vision - Required for all new students, and students entering Kindergarten.
- Illinois State Dental - Required for students entering Kindergarten, 2nd, and 6th grade.
Athletic Forms
All participants will learn the fundamentals of each sport and be given the opportunity to cultivate strong, healthy and disciplined minds and bodies. Participants will also develop a sense of good sportsmanship, fair play, and team spirit, as well as facilitate and enhance the moral and ethical development of all those involved.
